Looks like cong abnormality of 1st and 2nd arch, need a HRCT temporal bone to llok for other features : Dr Karthikeyan
It is a second arch defect: Dr Sunil Katuria

could be Johnson Mcmillin Syndrome
though appears to be congenital anomaly of 1st & 2nd arch needs thorough examination and investgations
