This is for the information of all colleagues that "ENT Surgical Update -
2012" will be hosted at Postgraduate Institute of Medical Education and
Research, Chandigarh on 22nd to 23rd September, 2012.

It will be preceded by "Hands on Cadaveric Dissection of Endoscopic
Sinus Surgery (Basic & Advanced Course) on 21st September, 2012.
Each delegate enrolled for cadaveric dissection would be provided
with a separate set of endoscopic instruments, camera, monitor and
microdebrider. The distinguished ENT surgeons from all over the country
and abroad have been invited to participate and interact, which will be
beneficial for the young Otolaryngologists.

This event has been organized on the request of colleagues and friends
from all over the country in response to a successful ENT Surgical
Workshop : An Update 2011 attended by more than 1100 delegates,
in which 14 live surgical demonstrations including cochlear implant,
endoscopic repair of CSF Rhinorrhoea, optic nerve fenestration,
DCR thyroplasty, thyroidectomy and other surgical procedures were
demonstrated. This year, we promise you an excellent academic feast
and live surgical demonstrations.

An early registration will help us to organize the event better. The
registration fee for delegates has been fixed as Rs. 2000/- to be paid in
the form of demand draft payable at Chandigarh in favour of FESS
Workshop.

Double Grommet Insertion

A 60 year old lady presented with left ear blockage and decreased hearing. She was having OME and was started on management of the same with conservative approach. There were no nasal symptoms. After two weeks, there was no improvement, but infact worsening of retraction so a recommendation for grommet insertion was made. She was lost to follow up.
She presented again three months later with almost atelectatic ear drum with a small air pocket anteriorly and fluid in area of OWN and RWN. An antrioinfrerior quadrant grommet insertion was done. However, the fluid in the posterior mesotympanum did not drain out as there were adhesions between the TM and the promontory that prevented drainage of posterior mesotympanum. I did a double grommet insertion taking care that the incision and the grommet did not disturb the ossicles. The fluid could drain immediately and through the myringotomy the adhesion between the TM and the promontory were broken using a right angle pick. A grommet insertion was done. The TM seemed stable after two months.
Please share how you would manage this. (I am expecting brickbats but please don’t strip me of my degree)

Abstract
Objectives: To assess whether the use of ice-lollies after tonsillectomy with or without adenoidectomy in children aged 2-12 reduces pain in the immediate post-operative period. Design: A prospective, randomised, single-blinded study design consisting of two groups with an intention to treat analysis. Setting: Tertiary referral centre. Participants: Children aged 2-12 undergoing tonsillectomy with or without adenoidectomy. Main outcome Measures: Pain assessment by nursing staff in the form of the validated modified Children's Hospital of Eastern Ontario Pain Scale at 15, 30 and 60 minutes and 4 hours. Results: Ninety-two patients were recruited into the study with 46 allocated to receive an ice-lolly and 41 not to receive an ice-lolly after exclusion of those with incomplete data. The 2 groups were comparable for number, age, sex and diagnosis. The pain score at every time interval was lower in the group that had received the ice-lolly compared with the group that had not. This was statistically significant at 30 (p= 0.008) and 60 minutes (p= 0.049) . Conclusion: Our data suggests that ice-lollies are a cheap, effective and safe method of reducing post-operative pain up to one hour following paediatric tonsillectomy.

- Suri Prabhu according to a colleague in Cochin - this is a good thing (monetarily) for the doctor - because the ear (and the attached patient) will have to visit the Ear specialist at least 5 or 6 times a year for the rest of his life - and each visit will be a standard protocol of -
  1. chemical cautery with phenol, or AgN03, TCA, or something like that-
  2. prescription of 2%acetic acid drops for 2 weeks
  3. when the patient asks - Doctor, why am i having this severe itching and fungus infection ? It was not so bad before the operation?" - the answer given by doctor is " Ear Drum is 100% intact - some red spots on the drum are causing the problem, but please do not use ear-buds"
  
  after about two years - the ear (and the attached person) get sick of the whole thing - and decide to visit that other "good" ENT doctor -
  and the original ENT doctor is happy and unhappy-
  
  happy because that damn ear is not interfering with his concept of 100% success in tympanoplasty-
  and probably unhappy because of the loss of a steady source of income-

I told them entire TM should be excised and grafted. Initially people laughed at me. But I got support from unexpected corner. Prof. Mirko Tos, who is around here on facebook, got up and said, What I said is making sense. The mucosalisation of the lateral surface of the TM is troublesome. The mucin is highly irritant to the EAC skin.

- Harpreet Singh Kochar It continued till our times too at institute and showed benefit. I practice this and chemical cautery. But am not facing this problem with graft now. The only thing is that GVP is sometimes messy as it tends to stain the skins and clothes and on occasion crusts form under which disease persists for which a saline irrigation and cleaning may need to be done weekly.

We are very sad to report the death of Professor Wolfgang Draf of Fulda, Germany after a long battle with cancer. Professor Draf was one of the leading Otolaryngologists of his day and exhibited outstanding surgical skill in all areas of the speciality. He was equally at home as an otologist, head and neck cancer surgeon, skull base surgeon and facial plastic surgeon. He was a committed anglophile and contributed immensely to academic otolaryngology in the UK as well in Germany and worldwide. He will also be remembered for his irrepressible sense of humour. He lit up any company that he found himself in. He had many friends in the British Skull Base Society and we are the worse for his death.

Abstract
After functional endoscopic sinus surgery (FESS), nasal packing may be necessary and a packing material which has benefits in both cost and efficacy would be required. This study aimed to determine the efficacy of Gelfoam packing on hemostasis and wound healing after FESS. Patients who underwent bilateral FESS due to chronic bilateral rhinosinusitis were enrolled. Randomly, one side was selected for Gelfoam packing and the other side for no packing. Subjective symptoms and objective findings such as synechia, granulation, pus discharge, edema, stenosis, and crust were evaluated. A total of 21 patients (17 men and 4 women; mean age 39.7 years ranging from 12 to 75 years) were included. There were no statistical significant differences between two groups regarding both subjective symptoms and objective findings during 4 months after surgery. Three patients had postoperative bleeding in the no packing side. Gelfoam packing may be recommendable in terms of efficacy and cost-benefit after FESS.

Shruti ENT Hospital & Cochlear Implant center on the eve of completion ocf 50 succesful cochlear implant surgeries in the period of 3 years announces its first Live Ear Surgery Workshop on the 27^th & 28^th of January, 2012, to be held at the hotel Gateway – Taj, on the banks of river Tapti.

The workshop is an excellent combination of live surgical demonstrations, lectures and panel discussions amongst the most eminent otologists from across the country. We have amongst us Dr Robert Vincent, from Causse clinic, France who will be demonstrating for the first time in our state – his renowned silastic banding and malleus repositioning techniques apart from laser assisited stapedotomy. We are also fortunate to have padmashri Dr Mohan Kameswaran as the course director, who not only would demonstrate surgeries but also take part in educational panel discussions.

Abstract

To evaluate the effects of open rhinoplasty incisions on tip projection using digitized photographs. Thirty-one patients, who underwent open technique rhinoplasty were prospectively included in the study. The lateral aspect photographs were taken before the operations. Following midcolumellar incision septal elevation was done until septal cartilage was shown. After replacing the skin totally back and suturing midcolumellar incision, the intraoperative photographs were taken. The projection indexes were measured by Goode method from the photographs and the measurements were compared. A statistically significant decline of the nasal projection was established after open technique approach. Open rhinoplasty approach led to the decrease of the nasal tip projection. This result was thought to be the effect of ligamentous disruption.

Abstract

This study documents the speech and swallowing outcomes of isolated ipsilateral cricothyroid approximation (aka tensioning thyroplasty; Type IV thyroplasty) for the treatment of high vagal paralysis (combined superior laryngeal nerve and recurrent laryngeal nerve paralysis). This is a pilot study of five cases with high vagal paralysis consequent to skull base neoplasms. Unilateral cricothyroid tensioning sutures were used. In all cases, vocal fold tensioning and vertical realignment of lax vocal folds were achieved. A partial, but acceptable medialization of vocal cord position was achieved. In all cases, aspiration was minimized and normal swallow function was restored by 6 weeks. The voice outcome was excellent in four cases and acceptable in one. Cricothyroid approximation restores vocal fold tension; in addition, it restores vertical vocal fold position and partially restores horizontal vocal fold position. Good voice and swallowing outcomes have been achieved. The procedure is quick, safe, and convenient when combined with a skull-base excision procedure. Further evaluation is merited.

A 7 yr old child accidentally swallowed a one rupee coin. It was already in the stomach when i first saw him. Now it is more than 3wks, child is asymptomatic,coin is somewhere in small intestine, paed surgeon do not want any intervention. I want to know ur experience as to how long u have seen coins staying inside and then passing out on its own.
Child is on a normal diet.

- Harpreet Singh Kochar have followed up patients who had swallowed coins and toy car wheels and adults with tooth caps and bridge.. all came out.. the narrow parts are obviously ileocaecal junction and pylorus.. need to get serial abdominal xrays daily till it clears.. usually in two days..

- Shashidhar Tatavarty almost every single day of my residency passed on seeing children and coins, so much so that we did know the age and the denomination of coin and pedict the outcome.. please remember , if child has adhesions or polyps/ or scarred ileoceacal valve ( recurrent appendicitis) in intestine the coin gets impacted. the longest you can wait is 4 weeks (yes it is that much and blunt fb like coin does nt mean universal impaction). also if child is having constipation the stool softners are recomended. if child gets cramp, the likely hood of impaction at valve is more and he should get buscopan or similar antispasmodics.. warning signs ae feve, vomiting and tahycadia and perf/ peritonitis has to be suspected.. CT is the best modality to investigate and instead of laparatomy endoscopic retrievel should be done by peads gastro...

Ninad S Narkar How much was the compliance? Can think of congenital ossicular fixation if compliance is low. Glue was probably coincidental.

Gowri Shankar may be the grommets are non functioning and blocked, taking that all the tests are done well

Saket Aggarwal almost 6 months now, had effusion. grommets are working well. checked under microscope. TM is normal now.

Suri Prabhu yes - Saket Aggarwal - you must be this childs second or third doctor - and, the first doctor would have put in grommets (in the antero-superior segment) - and the resulting tympanosclerosis would have fixed either the malleus itself, or the mallear ligaments - the child needs to subjected to a siegle examination (which is not very difficult with a nine year old) - and you will find that (probably) the malleus does not move - confirm by direct palpation - and if that is the case, the next action is predicated -

Suri Prabhu and likely - that the hearing loss is not equal - more like 20 db in one, and 40 db in the other (with both thresholds not improving after grommets)

Saket Aggarwal Dear Suri Prabhu, unfortunately, I am the first surgeon, and I have no obvious explanation for the outcome. Hearing loss is equal in both ears

Harpreet Singh Kochar Long standing OME tends to cause adhesions in ME leading to entrapment of SSS. Would give you a B curve and as obvious the audiometry would show a CHL less than complete osscicular fixation. in this case a 40 dB loss sort o fits into picture. One other possibility is of nitrous pushing in the glue to posterior mesotympanum if the same was not switched off during surgery. The gelatinous glue tends to persist there. I would do a HRCT to evaluate further

Harpreet Singh Kochar During grommet insertion excess bagging by anesthetists needs to be checked. Tends to displace glue from ME and the whole purpose is defeated.

Ravi Sharma very interesting. How long did the child have a hearring loss for, before surgery. I suspect ossicular fixation / tympanosclerosis or ME adhesions. Suggest doing a Tympanotomy +/- CT scan.

Mohamed Mahaboob Shareef What did the child present with? Is it deafness/ speech problems. If it is persistent deafness , could it be intra op damage to ossicles ?

Ajit Man Singh: to add academics to this: role of systemic antifungals? role of additional antifungal/steroid ointmetn to reduce associated otitis externa?, role of the age old GV pain?

Chandrashekhar Tengli: I generally clean of and apply Betnovate-GM locally. If i feel there is need, i prescribe them antifungal+steroid drops..I am good results.

Vinod Felix: there is a good no of cases due to long term usage of antibiotic steroid drops..which pts get OTC& from GPs

Chandrashekhar Tengli: agreed sir@ Dr.Vinod Felix. Also the season,rain followed by bright sunlight creating perfect environment to grow in the ear!!!

Rajendra B Bohra: Monsoon has extended and so we are getting lot of cases of Otomycosis..because of humidity...they are keeping us busy and giving us sense of being busy practitioner...so make the hay while sun is shining( or is it raining..)....it has become bread and butter for now...

DrRajeev Adhana :suction and oint. is best ....drops increase the problem due to moisture .

Sanjay Sood: GV paint is a good option for the non responsive to antifungals and also in recurrent otomycosis but it leaves a stain. Also instruct the patient not to keep the ear plugged

Vikas Agrawal: fungus grows on dead keratin of ear canal,with moisture...i always remove all the keratin layers under microscope with suction,then aft checking integrity of TM, pour surgical spirit,(which burns also apart from making patient giddy)and prescribe candid -o/otek ac type of ear drops to be filled in the ear canal 4 times a day for a week-10 days(they require at least 5-7 bottles)with use of hair dryer.....recurrence rate in rains in mumbai (moist)can be otherwise very high

Vikas Agrawal :never syringe,as many a times epithelium is alrady damaged and forceful syringing can cause perforation.......moisture any way has to be avoided
